Antifreeze drips from here?

Whenever I give around half throttle of or more antifreeze leaks and causes smoke. It looks like the leak is coming from the black ring attached to the rusty looking pipe to the left of the heater core inlet/outlet pipes. Can anyone tell me what this is and if it's hard to replace? I'm just hoping it's not something else to do with my heater core. Thanks guys in advance. Click image for larger version

Also seems to happen when I idle with the heat on, any advice would be helpful.

Looks like a bad / split heater hose. Judging my the clamp, it should be replaceable from under the hood.
