1995 Mustang 351W Throttle Cable Help

Hey I'm new to these forums, but I have a 1995 mustang with a fuel injected 351W, and the throttle cable pulls out of the gas pedal in between shifts when I really get on the throttle, or sometimes if I just really get on it. At the top of the gas pedal where the cable mounts is a U shape and the cable has a rubber bushing that fits down onto it. Do you guys have any suggestions on an easy way to fix this? Anyone else have problems with the throttle cable coming off the pedal? Thanks in advance.

Never ran into that problem before.
Can you get a picture or two so we can see exactly what the issue is?
