I noticed the same issue after my first gas fill up. It's very picky with different style pumps. I now only fill gas at the same station and the same pump all the time. As far as gas cap, it doesn't come with one but you can buy one off amazon. It's a ford product so it works perfectly fine. It's an actual locking cap with a key.

If you stick the nozzle too far in I noticed it affects the venturi and causes this. Also here is the locking gas cap,

I had the same issue when I got my 14, but I found the sweet spot to place the nozzle and it works fine now.
I've also had the locking gas cap for over 6months..................although I bought it from dealer at this price, if you need any OE Ford parts this is the place to go as they have the best prices of anywhere on the net. I got my a GT500 dual muffler kit with rear valance, both mufflers, and all the hardware for $625 shipped to my door on the West Coast. AM and CJ Pony want $800.

I have never had this problem (scratches head). And I get gas 2-3 times a week. Wonder why? Where are you guys located? Maybe it's something on the pumps in the states you live in?

Mine occasionally does it, depending on which gas station and pump style. My wife's 2013 Fusion does it bad, every single time. I haven't worried about gas theft from no cap, milk and alcohol is more expensive and nobody is breaking into my house stealing those.
Also it's hard to try to pour something in to maliciously contaminate your tank unless you use the special white Ford funnel.
Would be nice though if the fuel door locked when your car is locked.

Are you guys having problems using those hideous vapor recovery pumps? Those can be finicky at times.

I've had locking gas caps for over a decade and now that premium gas is $3.00 per gallon once again, I don't take chances.
It's not that I don't trust people I know, it's the ones I don't know. Reason I have a CW permit and a no trespassing sign that says, "We own weapons and a backhoe".

I live in California and have experienced the same issues 2 times over the last three years. The last time it happened was two weeks ago and it actually poured out under the car through the vent or overflow tube.

Mine fills fine (with vapor recovery in NJ), but if you try topping off after the first click, it just spills all over the ground, from somewhere well below the filler.

If you ever get under your car there is an overflow tube that comes out on one side or the other of drivers side muffler......I've often wondered if this is partially plugged, could that cause the problem, since it is behind the rear tire.

Next time I have er up on the lift, I will take a closer look. Might be soon as I'm pretty unhappy about my spongy brake pedal and I may just bleed them myself. It is real hassle to get to dealer when 95% of the time they fix nothing or give the "can't duplicate" line of crap.
