P2195 code on 2014 Mustang GT

This is the 2nd time I have gotten this code bank 1 sensor 1 stuck lean I replaced the oxygen sensor about 6,000 miles ago and the issue went away it's now back again same code when looking at datalog I notice the lamda stays .98 or lower for bank 1. I have Jba longtubes headers off road h pipe and jlt air intake along with jlt oil seperator I have the car tuned by Bama performance i had a few issues when they first sent me the tune back when my first o2 sensor failed but since then I no got the tune corrected feels like it makes good power but 3 days ago the p2195 error came back i bought another 02 sensor and cleared the codes and the lamda voltage is back in sync with bank 2 sensor 1. What could the issue bad tune? Anyone else having this issue?

I would get a revised tune. Are you using o2 extensions?

No extensions and I have data logged with Bama several times they are requesting more logs of cruising. I understand a seams or failing but the factory sensor went out around 8,000 and I bought another oem sensor and it lasted 3,000 miles I'm now on a 3rd sensor. Im running jba titanium headers I notice they get cool quickly and I'm wondering if this is causing the issue. If I cut the car off and let it set for 5 minutes I can start it back up and the temp is less than a quarter of the way to normal
If I start driving it will start running poorly around 3,000 to 4,000 rpm when taking off once it's up to normal temp it will run great. I just can figure out what is causing the sensors to fail.

Are the sensors in the 3:00 and 9:00 position? If so you'll keep blowing sensors until that's corrected. It's a common problem.

I think the JBA comes in about the 9 o'clock position but how come its just Bank 1 giving me issues bank 2 is in the exact same location but facing at 3:30 or 4 o'clock position. Anyone else running JBA longtubes that have the exact same issue?

Did you relocate your O2 bungs I believe they have a location I can use in one of the primaries however I know it's only monitoring the one cylinder

If they need to be in the collector then what's the solution I don't think I can relocate the 02 higher on the collector since the o2 sensor will hit the body
