Water has to come from somewhere to get in the oil. If coolant is disappearing, the other answers are good - the next step is to pressurize the cylinders one at a time to see if there is a breach between the cylinder and the coolant jacket. If coolant is not disappearing it is not getting in the oil (I canna' change the laws of physics) and the most likely source is condensation from poor crankcase ventilation.

Blown head gasket, cracked head, cracked block, timing chain cover/water pump gasket leaking into crankcase, or intake gasket leaking at cylinder head/intake/water jacket junction leaking into lifter valley......
